Instructions for Completing
- Complete all blanks. A good source of information is your check stub detail. If you don’t have all of the information, you may call 1-800-292-3133 and speak to one of our customer service representatives.
- If more than one property is included in the transfer, you may include an attachment that includes all the requested information on the Model Form Division Order.
- Once you’ve completed the blanks, print the form for the buyer(s) to sign.
- Mail to Flint Hills Resources, LP; ATTN: Ownership Services; P.O. Box 2917; Wichita, KS 67201.
- BE SURE TO INCLUDE A COPY OF THE RECORDED CONVEYANCE DOCUMENT.
Flint Hills Resources, LP reserves the right to require changes to the effective date if such date is past, and Flint Hills Resources, LP has not been notified in thirty (30) days advance of the sale. Retroactive payment adjustments will be the responsibility of the owners.
Definitions Used on the Model Form Division Order
Property Number - The property, lease or division order number used by Flint Hills Resources, LP that appears on the check detail when payment is made to the payee.
Effective Date - Identifies the earliest date for which this division order will be effective.
Property Name - The property, lease or well name that is covered by this division order.
Operator - The operator of the property, lease or well covered by this division order.
County/Parish and State - the county or parish and state where the property, lease or well is located.
Property Description - The description of the property from which proceeds from production are being paid pursuant to this division order. (Be as specific as possible.)
Production - The type of production (oil, gas, other) that is covered by this division order.
Owner Name, Buyer - The person or entity acquiring the interest covered by this division order.
Owner Name, Seller - The person or entity selling the interest covered by this division order.
Owner Number - Flint Hills Resources, LP’s identifying number for the owner. This should be the owner number that appears on the owner’s check detail. The Buyer may not have an owner number if he has never received a check from Flint Hills Resources, LP.
Interest - The decimal interest that is being conveyed. Please use the decimal interest as it appears on your check detail. Do not use fractions.
Interest Type - The interest type that is being conveyed. Royalty Interest (RI), Working Interest (WI), Overriding Interest (OR).
